She is a nationally recognized palliative care and end-of-life expert who designs and oversees the implementation of community projects. Prior to her work at Excellus Health Plan, Inc., she was engaged in private practice in Internal Medicine and Geriatrics in Rochester, New York.

Dr. George Giokas is the director of palliative care for The Community Hospice. Dr. Giokas is a Consulting Staff Member at Ellis Medicine and Sunnyview Hospital, and member of the Ellis Ethics Committee.
Joan Dacher, PhD., MS, BS, RN, GNP
Dr. Joan Dacher is a Professor of Nursing and was the founding Director of the Nursing Science Program at The Sage Colleges. Joan has worked in the field of Gerontology and End of Life care for 27 years. She holds a PhD in Gerontology and Health Policy from the Nelson Rockefeller College of Public Affairs and Policy as well as a postdoctoral certificate from Harvard Medical School in Palliative Care Education and Practice. Joan previously served as the Director of the Palliative Home Care Program at The Community Hospice. Joan serves on the Board of Trustees of HPCANYS is the Chair of HPCANYS Palliative Care Advisory Committee.
