How to Request a Receiver


Individuals currently registered with the NYS Commission for the Visually Handicapped or the Library of Congress Talking Books Program are automatically eligible for RISE.

Applicants not registered with either of these services must have a visual or physical condition which impedes access to printed material. This condition must be certified by a physician, nurse, social worker, librarian, rehabilitation counselor, or appropriate professional.

If you qualify, we will loan you a receiver at no charge. To apply, please complete and submit the application form.

Note - you will need Adobe Acrobat Reader to download the form. If you do not have Acrobat Reader, you can download free software at this site.

How to Return a Receiver

To return your receiver, simply put it back into the original box it was mailed in and send it to:

The original box will have a sticker on the upper right hand corner that reads "Free Matter for the Blind and Handicapped."

With this sticker on the box there is no charge to mail it back to WMHT-RISE. All matter marked in this way is mailed free of charge.

If you do no have the original box, another box can be substituted and you may request a "Free Matter for the Blind and Handicapped" sticker by calling the RISE office at 518-880-3436 or 518-880-3400.
